Throughout the discussions in this course we will be focusing on one single condition; gradually unfolding week by week how this condition can grossly affect the entire human body and its systems.

The goal of the course discussions is primarily, to build upon the skill of differential diagnosis and secondary, to give a clear view as to how a single pathological process can possess the ability to affect an entire human organism.

Each week you will be presented with varying components of a new system being adversely affected by the unidentified condition. Your task as a healthcare professional in training, is to thoroughly research and envelop yourself within the process of differential diagnosis to arrive at a definitive diagnosis at the end of the course in week 8.

In phase one of the discussion, you will be tasked with presenting your researched information into a video or written presentation answering the questions below, while also considering your response and symptom presentation from weeks 1, 2, 3, 4, 5 and 6.

Written presentations should be a minimum of 2-3 paragraphs per each diagnosed condition (3 conditions minimum).

Symptom Presentation:

  • Adrenalin: Increased epinephrine (catecholamine production) from the adrenal medulla
  • Heart Rate: Increased resting heart rate with intermittent bursts of arrhythmia
  • Blood Pressure: Systemic hypertension reported over the past 8 months
  • Hypertensive Retinopathy: arteriolar constriction, vascular wall changes, cotton-wool spots, yellow hard exudates, and optic disk edema

Previously, you researched and considered three conditions through the process of differential diagnosis that would present with varying abnormalities in homeostasis, metabolism, triglycerides and DNA in week 1. Abnormalities in oxidation, plasma and tissue enzyme activity, inflammation and alopecia respectively in week 2. Increased cortisol and demonstrated bone loss in week 3. Fibromyalgia and muscle atrophy in week 4. Increased glutamate and memory loss in week 5 and neurogenic inflammation in week 6. She is ready to learn because of her willingness to get better.

The learning objective is to understand the parts of the incentive spirometer and what the mean. There are printed materials to follow along. The measurement is when I can ask my mom what all the parts are and what do they mean. The teaching strategy is understanding the printed materials and self-teaching. Taxonomy is the cognitive domain with comprehension.

The next learning objective is watching a video online demonstrating the use of the incentive spirometer and my mom will be able to demonstrate to me on how to use the incentive spirometer. The measurement is how she demonstrated the use of the incentive spirometer after watching the video demonstration and doing a return demonstration. This should include proper position, holding the incentive spirometer correctly. Sealing your lips around the mouth piece and breathing in slowly to the correct goal number on the cylinder. Taxonomy was psychomotor domain and to demonstrate.

While discussing my evidence-based practice change of early mobility with my mentor, we discussed many financial, quality, and clinical aspects of my project. The aspect that may prove to be the most difficult in my project is the financial aspect. In order to properly implement a protocol on mobility of ICU patients, education is a priority. As an ICU nurse, if my manager told me we were now expected to engage in early mobility of our patients I would be terrified. My mentor and I discussed the needed education and agreed that having a physical therapist spend some time with nurses on developing mobility plans would be the best way to demonstrate mobility expectations. This may require additional pay for the nurses, and possibly physical therapists. Along with hands on education, having nurses on both day shift and night shift become mobility champions would provide needed resources for the nurses. This too would require additional pay for the education time spent in training. While there would be some upfront costs associated with the implementation of my project, the end result would produce improved patient outcomes and a decrease in costs due to a reduction in hospital length of stay for the patients involved. According to Ronnebaum, Weir, and Hilsabeck (2012), an early mobility protocol resulted in a savings of $22,000 per patient (Ronnebaum, Weir, & Hilsabeck, 2012).

The Institute of Medicine defines quality as “the degree to which health care services for individuals and populations increase the likelihood of desired health outcomes and are consistent with current professional knowledge” (Agency for Healthcare Research and Quality, 2012). Improving patient outcomes, such as reduced time in ICU and in the hospital, is one quality aspect of my project. Implementing early mobility directly impacts quality by reducing complications such as DVT, pressure ulcers, and muscle wasting seen in bedrest.

The clinical aspect of early mobility in ICU patients will probably be the most challenging part due to the assessment and change in practice needed. There are many barriers associated with my evidence-based practice project such as safety concerns, time constraints, and availability of support staff. While discussing this with my mentor we agreed that education and support from the physical therapy department and physicians will be an important factor in the clinical changes needed to implement the plan. Early mobility requires nurses to mobilize patients either by range of motion, assisting with in-bed activities, and even assisting with out of bed activities such as walking. At Banner Estrella most critically ill patients are on bedrest, so this may be a challenging clinical change.

Sepsis is a major cause of hospitalization and death throughout the world, the hospital I work out has a screening process that we as nurse must assess the patients every 4 hours for signs of sepsis. The symptoms can be easily overlooked, and death rates continues to increase for septic shock. The point of the screening is to recognize early signs of septic shock and initial treatment right away. Patients, BP, heart rate, respirations, temperature and wbc indicates to the nurse if patient has a positive screen for sepsis, once the test is positive the nurse must obtain a lactated acid, blood cultures, and informed the doctor.

According to Birriel, 2013, Early treatment of sepsis, severe sepsis, or septic shock with quantitative fluid resuscitation has been shown to improve patient outcomes in multiple studies,(4,5) as has early treatment with antibiotics(5-8); however, to attain the greatest benefit from these therapies, sepsis must be identified as early as possible in its course.

Few years ago, the facility that I work wanted all nurses to do hands off communication at bedside.  A lot of nurses were very reluctant to this change.  At some point the facility became very serious about it and wanted all the nurses to implement it. Now not only we have to give report at bedside, we have to wake patients and families up, so they can participate.  I found it very interesting and proficient.  It takes less time to give report, good for patient safety, patients and families are less anxious and ask less questions afterward because they know the plan of care for that specific day, and help nurses not to forget to pass on any relevant information to the patients’ cases.  This is what two authors have to say about bedside report:  “In traditional shift to shift reporting, nurses spend the end of their shift (and often into overtime) transcribing or taping a report for the oncoming nurse, who then spends the first portion of his or her shift reading the notes or listening to the tape.  Bedside shift reporting saves time and allows the incoming nurse to ask questions. It also improves patient safety by involving the patient and ensuring patient and caregivers are on the same page.” (Hendren, 2010).  “Conducting nurse-to-nurse shift report at the bedside, in the presence of the patient puts the patient central to all care activity information (Anderson & Mangino, 2006).  It allows the introduction of the incoming nurse by the outgoing nurse to the patient and the family.  Being in the room, helps incoming nurse to ask questions, visually see all medicines that the patient is having.  Bedside reports also provide the opportunity to reassess the patient’s goals from the prior period and update the goals as needed. Anderson and Mangino (2006) observed increased patient, staff, and physician satisfaction as well as financial savings after implementing bedside shift report.” (Manning, 20036).

community perception towards nurses and the nursing profession

Question description

This course has been amazingly interesting to me and I quite enjoyed seeing the theorists behind my every day nursing practice. I do not I think I realized how hard the nurses before had to work and advocate to ensure our patients were receiving the best care, and to ensure nursing was a valued and respected profession. We have made significant strides since the times many of these theorists were implementing their ideas, and modern-day nurses have these nurses to thank! It is because of them I am able to work in a safe environment to provide safe care to my patients, and that nurses are now respected to speak up and advocate for the health of our patients. They allowed us to actually care and feel with our patients instead of just doing tasks directed by others. I love my profession, and these theorists made nursing the way it is today. This course has given me more respect for this profession and it has given me new insight into how I treat my patients and why my nursing practice is the way that it is. Hopefully it has inspired some to create new nursing theory and to continue to advance this profession!

According to Colley, nursing theory gives nurses a sense of identity, and a directed way to care for patients; it allows nursing to be viewed as a unique profession and shows nurses can greatly contribute to healthcare (Colley, 2003). I do believe I have met the objectives of this course. I understand how these theories set the foundation for nursing to be multicultural, caring, and a profession of advocacy. I understand how patients are not just their illness, but parts of health and illness. Nursing theorists have allowed current nurses to advocate and educate our patients, ensuring we are preventing further instances of illness. This course has allowed me to think and view how these theories apply to my current practice and how I can better for my patients in my day to day work. I am able to think about their theories and I have been finding myself spending more quality time with patients due to learning about these theories. I found the importance of this class to be more one of caring and on a deeper level than just nursing skill and tasks. In my associate degree program, the goal was to ensure I could perform the tasks in order to safely be a nurse. This course has allowed me to view nursing in a broader light and to be able to focus on the critical thinking aspect of the profession and made me think of how I can better forward this profession.

Justify for the facility’s stakeholders the use and placement for each piece of play equipment, toy, and furniture. Make it clear when and why you would need to divide some of the space to separate children in early childhood from those in middle childhood. Be sure to specify:

  1. How does each piece of play equipment, toy, and furniture contribute to the four types of development (e.g., fine-motor movement helps physical growth)?
  2. How does each piece facilitate social-emotional bonds of children with their families?
  3. How does each piece improve coping and adjustment to long-term care?

Paper Requirements:

  • Your paper should be seven- to nine-pages (not including the required title or reference pages) and double spaced.
  • The paper should be based on references to scholarly materials (rather than on introductory textbooks, popular website writings, or musings, for example). To that, your paper should synthesize current, relevant research on your topic from at least five scholarly sources. Cite and reference these sources according to APA style.
  • Include an introduction that provides a clear statement of your position on the topic, a body that discusses the implications of possible solutions for the project under consideration, and a conclusion regarding how best to manage the issue (i.e., your informed opinion on the issues raised).

The paper must demonstrate what you have learned about current issues related to human development. Your paper is expected to have depth and offer a thoughtful analysis on the topic. The most common mistakes in writing an essay are (a) simply summarizing the topic and not analyzing it or discussing your views and (b) failing to relate the subject to the broader context of restorative justice, as discussed in class and as found in the readings.

Adequately including the broader contexts includes, for example, considering how your research might yield a different approach to restorative justice in the United States.
